The attached essay is my original work and all other personal information is true. I understand that the essay and application becomes the property of Advia Credit Union and the Advia Foundation, for whatever use they deem appropriate. Such use by the credit union shall not violate any applicable law. I agree to abide by the decision of the Scholarship Committee. I agree to cooperate with Advia Credit Union/Advia Foundation in promotional activities related to the scholarship competition, and should I be selected a winner, give them permission to use my name, quote, and photo in promotional activities.

The Advia Foundation reserves the right to modify or eliminate the Scholarship Program at any time without notice. Notwithstanding the foregoing, any award due and payable under the program prior to such modification or elimination shall continue to be payable according to the program’s terms in effect at the time the Advia Foundation made the grant. All entries shall become the property of the Advia Foundation. Funds forfeited if not used within 1 year.
